Annual Wellness Visits: Implementation Process

Providing annual wellness visits to patients covered by Medicare is a great way to grow your revenue year after year, whether you’re a practice of one physician or a large hospital system. Blue & Co. has developed a simplified implementation process to start performing annual wellness visits at scale to increase revenue in your healthcare organization. We advise healthcare leadership to follow four phases that range from reviewing current processes to surpassing financial goals.

Phase 1: Building Blocks

Phase 1 involves evaluating the current state of your practice, establishing a communication strategy, forming an AWV steering committee, and boosting your organization’s capacity for more annual wellness visits. Another key part of this phase is educating your Medicare patient population on the importance of receiving an annual wellness visit.

Phase 2: Stakeholder Buy-In

Phase 2 focuses on ensuring provider-friendly workflow, implementing electronic health record triggers, demonstrating downstream revenue benefits, and launching a pilot program. It is important to ensure key stakeholders and leadership understand the value and benefits of expanding AWV services.

Phase 3: The Roll-Out

Following the pilot program’s success, Phase 3 aims to expand the program to all providers and locations in your healthcare organization. Blue & Co. will work side-by-side with your organization as this program expands to ensure its effectiveness.

Phase 4: Monitoring

Finally, Phase 4 emphasizes ongoing success through regular reporting, provider feedback, and the development of audit tools to track the sustained revenue growth from AWVs. This plan can help a healthcare organization grow revenue in one year, but Blue & Co. will be your partner to ensure this plan continues to generate more revenue year over year.
